Named GP

From the 1st April 2015 all practices are required to allocate a named GP, who is responsible for patients' overall care at the practice, to all patients.

All of our patients have been allocated a GP during the last year.

If you are not aware of your named GP please contact the practice and we can inform you of who this is.

The surgery will close at 12:30pm for training on the below dates and re-open at 8:30am the following day;

Electronic prescribing service

You can find detailed information under the "Prescriptions" tab at the top of the page.

The Electronic Prescription Service(EPS) ia an NHS service. It gives you the chance to change how your GP sends your prescription to the place you choose to get your medicines or appliances from.

What does this mean for you?

If you collect your repeat prescriptions from your GP you will not have to visit your GP practice to pick up your paper prescription. Instead, your GP will send it electronically to the place you choose, saving you time.

You will have more choice about where to get your medicines from because they can be collected from a pharmacy near to where you live, work or shop. For more information ask at reception or vist, your pharmacy or GP practice.

New Doctors

We would like to let our patients know that Dr Deborah Miller has now left the practice for a new adventure. We would like to welcome Dr Mohammed Javid and Dr Sangeetha Ganeshamoorthy to the practice. Dr Javid has been working at the practice for a while as a locum and has now become a permenant member of the team in January 2016. Dr Javid was born and raised locally and studied medicine at The University of Leeds. He is married with two children and enjoys football and socialising.

Dr Ganeshamoorthy has also recently joined the practice in January 2016, who grew up in Canada and moved to England to pursue a career in medical training. Dr Gorashemoorthy lives locally with her husband and two children.

Please click the relevant tab and log in to submit a new patient registration form. Appointment booking, prescription services and access to results are also available by clicking the relevant icon and entering your username and password. New users can collect their personal log-in details from reception upon providing a form of identification.

Help us to update your medical record

It is good to keep your records updated with us so that we can offer the right care for you. Forms are available in the waiting room at the surgery.

You may have received a letter in the post from NHS England regarding ''. This is where minimal data will be extracted from medical records in order to improve patient care. Strict rules and guidelines are in place to maintain confidentiality and to ensure that patients will not be identified in reports.
